Virginia Rural Health Transformation Program - Request for Applications Open Through this RFA, the Thomas Jefferson Planning District Commission and the Northern Piedmont Regional Collaborative will competitively award funding to eligible organizations to develop and deploy mobile and hybrid care models that serve eligible rural residents within the RFA’s target rural counties.
